Many sites explain the basics of HTML forms, but I found these two to be the most useful:

• HTML Forms and Input - An overview from W3Schools. See the reference pages for each tag for more information and interactive examples.
• Accessible Forms - A good article explaining a few ways to make forms more accessible.
